diarrhea

I have a patient who suffers 4 weeks from watery diarrhea that did not respond to any antibiotics ( flagil , tarivid or ciprogis) In the CT of abdomen there were seen granuloma in the chest and hypodensed areas in the liver and lymph nodes in the mesanterium. blood test for blood count and chemistry were all right. what is the diagnosis .
