Diagnosis: Left breast cancer, invasive ductal carcinoma 16mm, 1/17 positive, ER+. BRCAl alteration highly likely to be cancer risk related. History: Mar 2000: Wide local excision and axillary dissection. Treated with adiuvant FEC chemotherapy x 6 courses, radiotherapy and Tamoxifen' April 2005: Completion of 5 years of Tamoxifen changed to Arimidex. April 2006: Baseline bone scan shows osteoporosis. Started on Fosamax. April 2008: Right pleural effusion with outside cytology suggesting relapse' Bronchial biopsy at Brompton revealed histoplasmosis. April 2008: Confirmed relapse on pleural biopsy. Commenced on Exemestane I understand Mrs Horsey saw you recently with problems of breathlessness' We reviewed her recent CT scan which was undertaken by you in November (report enclosed) at our x-ray meeting. We did not have the hard copy films from her previous scan available but compared to the report it appeared that her disease was stable. There is some right pleural disease and a small amount of fluid but no new problems.
