fever and diarrhea

Hi, My six and a half month baby is suffering really badly from vomiting, diarrhea and fever since friday night. We went to see the on call doctor which said she notices some redness in the throat and ears and prescribed antibiotics but we havent used it yet. It all started with runny nose and a mild cough and was followed by vomiting that stopped after 24 hours. Then came the diarrhea - now it is 30 hours since he first started with it. In addition he has around 39 degrees temperture. We use only paracetamol and extra fluid. Since it is almost impossible to see a pediatrician in real time here - london our only option is to go to casualty in hospital. Which circumstances do you advice us to do so ? Is there anything else you can recommend us to do ? We can read hebrew. Thank you very much
